Please be advised that before making an appointment, you must read the instructions, fill out the forms, sign them and send them to our email address. Otherwise, you can make a print of the form and sign it in person at our beauty salon. In this case, your eligibility will be confirmed on the day, or in a free consultation session. In either case, we cannot guarantee that you are eligible to do microblading services, before going through the check up phase.
